Informed Consent

1. I acknowledge that any information contributed by me is true to the best of my knowledge and that the present condition of the area that has been treated or will be treated is states on this record.

2. I absolutely understand and accept that the camouflage/inkless procedure is a process, often requiring multiple sessions to achieve desirable results and that 100% success cannot be guaranteed. I fully understand that Flashback Skin Camouflage only provides beauty services; there is no medical treatment involved.

3. I accept responsibility for my aftercare. I understand that I need to follow them in order to protect the colour of my camouflage/inkless tattoo and to prevent infection.

4. I understand that the colour selection and colour results in all procedures are not an exact science and that the right colour of my skin tone might not be achieved in just one session.

5. I understand that since the pigment used for this treatment is a skin tone pigment, this colour cannot be erased even by a laser treatment.

6. I understand that the healing speed and process may take a couple of months depending on my skin and that during the healing process my skin will turn red but will start to fade as its healing.

7. I understand that the colour of my camouflage will be darker than my skin stone while its healing which may take a couple of months.

8. I understand that the healing process may take up to 3 months and that it completely varies on my skin’s healing abilities, my aftercare, and my health.

9. I understand that a camouflage tattoo is a tattoo procedure that may or may not fade and that it might require a touch up over the years.

10. I realise that with any beauty services there may be certain risks which must be understood. I will be fully responsible for any and all results which may arise from these beauty services.

11. I do hereby agree to hold Brittany Gordon-Brown and Flashback Skin Camouflage free from any and all claims or suits for damages, for injuries or complications resulting from any beauty service provided by Brittany Gordon-Brown and Flashback Skin Camouflage..

12. The nature and purpose of this service, the risks involved, and the possibility of complications have been fully explained to me and I agree to go ahead.

13. I have been explained that I must complete all Laser/IPL treatments on my treated area prior to receiving treatments by Brittany Gordon-Brown and Flashback Skin Camouflage OR I agree to inform my aesthetician or anyone operating such a device that I have had such treatments (pigments in my skin). Failure to notify your beautician could cause you to burn or damage the end results of the pigments.

14. I consent to receive emails from Brittany Gordon-Brown and Flashback Skin Camouflage and permission to use my photos, anonymously (photos will only ever be of the area we are treating).

15. I acknowledge that I have read and understand the above and all of questions have been answered to my satisfaction and that I consent to have services provided by Brittany Gordon-Brown and Flashback Skin Camouflage in accordance with these terms and conditions.
